INR Goal 2.5 (2.0-3.0)

Post op SQ Heparin Until INR 1.8

Warfarin 5mg 1st Post op Day

ASA 81 mg/day

Mitral

INR Goal 3.0 (2.5-3.5)

Post op SQ Heparin until INR 2.0

Warfarin 5 mg, 1st Post op Day

Consider Lovenox if INR < 2.0

ASA 81 mg/day

Tissue Valves

Aortic

ASA 325 mg/day 1st month, then reduce to 81 mg/day

Consider Warfarin for three months with INR Goal 2.0-3.0

If EF< 35%, Atrial fibrillation, Thromboembolism

Mitral

INR Goal 2.0-3.0 for 3 months

ASA 81 mg/day indefinitely

Mitral Valve Repairs

ASA 81 mg/day for 6 months plus Plavix 75 mg/day for 6 weeks
